Refractive Eye Surgery – Advantages of Lasik Eye Surgery

Refractive eye surgery is very effective in rectifying visual refractive errors and decreasing the dependency on glasses and contacts.

It has been proven successful in reducing myopia (nearsightedness), astigmatism (elongated corneas), hyperopia (farsightedness) and many other problems caused by refractive errors.

The medicine comprises four types of refractive eye surgery procedures: corneal incision procedures; flap and photoablation procedures; implants and thermal procedures. Nowadays the modern technology allows the doctors to use the laser to reshape the cornea.

During a flap procedure, the surgeon cuts a tiny flap in the cornea to reshape the tissue underneath and correct the refractive error.

Lasik (Laser Assisted In-Situ Keratomileusis) is very popular in the field of refractive surgery and can correct astigmatism, hyperopia and myopia. At first the surgeon uses a IntraLase or a microkeratome to move a flap out of the stroma. Next, he uses an excimer laser to remove excess corneal tissue and only after that he replaces the flap.

The best candidates for Lasik are the people with flat or thin corneas. A small trephine blade cuts into the epithelium while a mild alcohol solution will smoothen its edges. Excess corneal tissue is removed with an excimer laser and in the end the flap is replaced.

Just like LASEK, the Epi-LASIK cuts the epithelium but epikeratome is used as a replacement for alcohol and blade.

The second stage in flap procedures is photoablation because it removes excess corneal tissue with ultraviolet radiation. The original laser eye surgery procedure was PRK (photorefractive keratectomy).

This intervention used local aesthetic eye drops to numb the eye followed by the reshaping of the cornea. The excimer laser is an ultraviolet beam of light controlled by the computer that won’t cause any damage to the tissue around the eyes.

Arcuate keratotomy and radial keratotomy are two corneal incision procedures that alter the surface of the cornea to correct refractive errors. Radial keratotomy (RK) makes a few spoke-shaped incisions in the cornea with a diamond knife and by doing that it can treat myopia.

Arcuate keratotomy is pretty much the same except that the diamond knife cuts parallel incisions to the edge of the cornea.

Farsightedness or temporarily hyperopic refractive errors can be corrected with heat thanks to thermal procedures. During the thermal keratoplasty procedure, the surgeon puts a ring of 8 or 16 small burns around the pupil. The heat causes contractions and makes the cornea steeper.

Thermal keratoplasty comprises Laser Thermal Keratoplasty (LTK) with holmium laser and Conductive Keratoplasty (CK) with high-frequency electric probes.

Last but not least, we have the implants. Thanks to implantable contact lenses (ICL) inserted in the cornea through small incisions, many patients suffering from astigmatism, hyperopia and myopia were cured. ICL sit behind the cornea refocusing the light and offering a natural image.

Traditionally, a LASIK surgery involves a microkeratome or a metal blade that cuts the corneal flap, which is folded back before the surgeon applies laser energy for reshaping the cornea.

IntraLase or bladeless surgery is also considered to produce a consistent thickness in the flap from one edge to the other, the accuracy of which has been unprecedented in flap creation technology. With a better flap stability demonstrated in bladeless LASIK surgery, the possibilities of flap complications such as partial flaps, free flaps, or flap dislocation can be reduced or eliminated to a great extent.

Although very rare, there is a unique risk associated with bladeless LASIK that might cause transient light sensitivity. Moreover, compared to a traditional LASIK surgery a bladeless LASIK might cost $300 extra per eye, which is quite expensive. According to experts, a traditional microkeratome can be very well matched with the finesse of the bladeless technology and thus, it depends on the individual choice to select between the two.

LASIK or Laser Assisted In-Situ Keratomileusis is the very latest in eye surgery. The correction of the visual imperfections is carried out within a computer and laser controlled environment.

LASIK or Laser Assisted In-Situ Keratomileusis is a procedure that involves the medical option of surgery to correct vision. The cornea is an important part of the eye that helps the complex muscle and nerve mechanism to focus light and create the desired image on the retina. The shape of the cornea and that of the eye are not perfect. This results in the image on the retina being blurred or distorted. The imperfection in the focusing power is referred to clinically, as refractive error. One of the three types of refractive errors is myopia. Myopia or nearsightedness creates a difficulty to focus on distant objects. The next is hyperopia or farsightedness, where the person has difficulty focusing on objects close at hand. The third is astigmatism. This condition causes distortion of the image on the retina because of certain irregularities.

Laser-Assisted Keratomileusis enables the surgeon to change the shape of the patient’s cornea, permanently. The cornea is treated with the help of an excimer laser. The micro keratome is the knife used to make the incision in the cornea. The stroma, which is the middle part of the cornea is then treated with the help of a laser vaporize that is controlled via a computer. The damaged portion of the stroma and the flap is replaced and this results in the enhanced vision. In LASIK surgery, the attempt is to remove of corneal tissue and reshape the cornea within a controlled environment.

Radial Keratotomy or RK is distinctly different from LASIK or Laser Assisted In-Situ Keratomileusis. In the former, a sharp knife is used to slit the cornea to change its shape via sculpting and using a laser. The major difference between the two surgeries is the way the middle layer of the cornea is exposed prior to being vaporized. While in Radial Keratotomy the top layer of the cornea or the epithelium is scraped to treat the stromal layer, in Laser Assisted In-Situ Keratomileusis a flap is cut and folded back. The LASIK or Laser Assisted In-Situ Keratomileusis procedure is the latest and tried and tested application in vision correction, the world over.

LASIK or Laser Assisted In-Situ Keratomileusis has now become the main surgery option that doctors use to treat myopia. However, prior to opting for the refractive procedure, it is essential to weigh the pros and cons on a tested personal value system, without the egging on of friends or the doctor. It is important to conduct a little research, either online or offline. There have been some patients who have lost vision that could not even be readdressed or corrected with glasses or contact lenses or recorrective surgery, after going through LASIK or Laser Assisted In-Situ Keratomileusis treatment. Some patients have gone on record with the development of glare and double vision and largely affected nighttime vision, post surgery.

Only a percent of patients achieve perfect vision post surgery and many still need glasses or contact lenses. The surgery could also result in ‘dry’ eyes that could cause great discomfort and reduce visual quality. In fact, it is as new as 1998, when the first laser was approved for the application of the LASIK eye surgery. The long-term safety and effectiveness of LASIK surgery is still being observed and recorded and is yet not known for sure. The Laser Assisted In-Situ Keratomileusis uses the innovation of the laser technology to reduce the use of additional surgical equipment and the time in the procedure.

LASIK or Laser Assisted In-Situ Keratomileusis involves the use of the computer in surgery and this adds to the precision of the operation and the quality too. The treatment is like any other option involving surgery and the results are also just as varied, person to person.

Advanced Laser Vision Correction Procedures

Whenever laser eye surgery or laser vision correction is mentioned, "LASIK" is often the next thought. Today, laser vision correction procedures are so technologically advanced that there are many options besides the conventional LASIK surgery.

Understand Your Options
For the individual looking to reduce their dependency on glasses or contacts, surgical procedures which improve the focusing power of the eye, are a tremendously effective option. The procedure commonly referred to as LASIK is the most technologically advanced way to correct vision, and has helped millions of people worldwide reduce their dependency on glasses and contact lenses.

With LASIK, an excimer (cool beam) laser reshapes the cornea, or surface of the eye, to alter the manner in which light rays enter the eye. This is the same principle used by contact lenses or glasses to achieve better vision. LASIK can be used to correct nearsightedness and farsightedness, and now can correct astigmatism as well.

LASIK is the most common term used to refer to laser vision correction procedures. All forms of LASIK use an excimer laser to reshape the cornea, but there are variations to the procedure which provide different advantages. A series of vision tests called Advanced Diagnostic Testing allows your doctor to determine which approach is best for you.

LASIK
LASIK (Laser-Assisted in-Situ Keratomileusis) gently reshapes the cornea with the use of an excimer laser. In traditional LASIK, an extraordinarily thin flap is created in the top layer of the cornea and then gently folded back. The underlying layer is then re-shaped with the laser in a matter of seconds, correcting the refractive errors that cause the vision impairment. The flap is then repositioned across the cornea, which naturally adheres and begins to heal within minutes. Of all the laser vision correction procedures, LASIK provides the most comfortable and rapid healing process.

Advanced Surface Ablation
ASA refers to the laser vision correction procedures that treat the surface of the cornea without the creation of a flap. PRK (Photorefractive Keratectomy) and Epi-LASIK are the two most common forms of ASA and refer to the methods of preparing the corneal surface just prior to using the laser. ASA is the safest form of laser vision correction. It allows a wider range of patients to be treated who may not be able to undergo a traditional LASIK procedure due to thin corneas, job restrictions, or other visual issues. ASA is also considered by many to provide the chance for the sharpest and clearest vision, since there are no induced refractive errors associated with a corneal flap. ASA does have a disadvantage because it usually involves a few days of discomfort (treated with Ibuprofen), and a slower visual recovery time, ranging from a few days to a few weeks. Generally, only the most experienced surgeons practice the Advanced Surface Ablation techniques.

Blended Vision
Blended Vision is a LASIK technique you might want to consider if you wear reading glasses or are nearing the age when reading glasses may become necessary. As we get older, we frequently have trouble reading, particularly small or very fine print. This is a natural part of the aging process associated with the lens inside the eye. When this happens, most people need reading glasses. If you use glasses to read prior to LASIK surgery or you remove your glasses to read, you may need to use glasses for reading after LASIK, because LASIK does not treat the lens inside the eye. However, a Blended Vision procedure may provide you with suitable near and distance vision. Blended Vision, also called monovision, is a technique in which one eye (usually the dominant eye) is corrected for clear distance vision, and the other for comfortable near vision. Blended Vision allows a person to see both close and distant objects clearly, though perhaps not quite as crisply as when both eyes are corrected for just one distance or the other. Many people who wear contacts use blended vision today and are very satisfied. Your doctor will demonstrate how this method works prior to your procedure, and thereby help determine if Blended Vision will work well for you.

Wavefront technology is now being incorporated to identify subtle visual distortions, which up to now have been unable to be measured, let alone be treated. Wavefront treatments such as CustomCornea provide the best opportunity for superior vision compared to that previously available through older, conventional methods of LASIK.
